Responsibilities:
​

Provide home based speech therapy for clients with developmental or intellectual disabilities.
-
Work on daily SOAP notes and complete quarterly progress reports for each client.
-
Implement speech goals and create individualized treatment plans specific to each client.
-
Provide speech strategies, give feedback and share progress for each client and their families.
-
Perform tasks described by the SLP supervisor.

Requirements:

A Bachelor's in Speech and Hearing Sciences, Speech-Language Pathology, Communications Disorders, etc.

AZ Dept. Of Health Services SLPA License
-
Reliable source of transportation with proof of car insurance and registration
-
Fingerprint Clearance Card
-
CPR certification
-
Arizona State license
-
Background check
-
One year of experience working as a SLPS preferred

Speech therapy evaluations for SLPA client.

Sign off on SLPA’s daily SOAP and progress notes.

Communicate and mentor SLPAs on client’s speech goals.

Direct therapy services to clients optional.

Provide consultations for both SLPAs and families in person when needed.
